This is a PRN role designed to provide coverage in the retail pharmacy at Lourdes Hospital.

Job Responsibilities:
The Pharmacist is responsible for delivering patient care by utilizing the nursing process, which includes data collection and assessment. This role involves identifying and prioritizing patient needs and evaluating the effectiveness of care provided.
The Pharmacist coordinates, provides, and directs patient education and collaborates with health team members to ensure comprehensive care.
The responsibilities include managing medication therapy to guarantee safe, rational, and cost-effective treatment while adhering to all applicable regulations and standards set forth by Federal, State, and accrediting bodies.
The Pharmacist is expected to maintain established departmental policies and procedures and comply with departmental objectives.
Key Duties:
- Collaborate with the interdisciplinary care team to conduct initial and ongoing patient assessments, analyze data, create care plans, implement treatments, and evaluate their effectiveness.
- Administer medications safely in accordance with state regulations and Mercy Health policies.
- Act as a liaison for patient care coordination across various hospital departments.
- Advocate for patient safety by engaging in continuous quality improvement initiatives within the department.
- Process medication orders and conduct reviews for appropriateness, safety, and efficacy, performing necessary clinical support tasks as required.
- Ensure accurate preparation of compounded and non-compounded products, adhering to safety regulations.

Qualifications:
- Graduate of an ACPE accredited School of Pharmacy with a BS or PharmD (required).
- Advanced pharmacy degree - Doctor of Pharmacy, Masters, Residency (preferred).
Licensing & Certifications:
- Current pharmacist license in the state(s) of practice (required).
- Valid state-issued ID (required).
- BLS Basic Life Support certification (preferred).
- Immunization certification (preferred).
Experience:
- Minimum of 2 years of recent experience in an acute care setting (required).
- 1 year of leadership experience (preferred).
- Experience in the field of pharmacy practice (required).
- One year of experience in the field of pharmacy practice (preferred).

- Monitor patient conditions during treatment.
- Administer non-intravenous medications.
- Communicate effectively with medical professionals regarding patient conditions and care.

- Provide medication education to patients.
- Demonstrate excellent customer service for both internal and external customers.
- Exhibit strong organizational skills and stress management techniques.
Training:
- Proficient use of EPIC Electronic Health Record (preferred).
- Exhibit proficient use of all computer hardware and software (required).
