Job Summary

Caregivers of Garden City is seeking a skilled Physical Therapy Assistant to join our team on a PRN 1st shift basis. As a Physical Therapy Assistant, you will play a vital role in delivering comprehensive physical therapy modalities to patients in their place of residence, promoting optimal patient outcomes and quality of life.

Key Responsibilities
  • Deliver accurate and comprehensive screens to patients, ensuring timely and effective care.
  • Obtain physician orders for certification and recertification of home physical therapy services, ensuring seamless communication with healthcare team members.
  • Ensure timely and accurate communication of patient clinical status to other healthcare team members and Agency staff.
  • Coordinate care planning and delivery with physicians, patients, and family/caregivers, other healthcare team members, and Agency staff to facilitate optimal patient outcomes.
  • Provide education to patients, families/caregivers, and other staff as appropriate, in physical therapy modalities as directed by the supervising therapist.
Requirements
  • Current valid state Physical Therapy Assistant license.
  • Graduate of an accredited physical therapy assistant curriculum/school.
  • Prior home health experience preferred.
  • Health certificate/physical examination and current TB testing.
  • Current liability insurance.
Language Skills

Able to communicate effectively in English, both verbally and in writing. Additional languages preferred.
