We are seeking a compassionate and skilled Licensed Practical Nurse to join our team at EverHeart Hospice. As a key member of our care team, you will provide high-quality patient care and support to individuals and their families in need of end-of-life care.
Responsibilities:- Provide direct patient care, including routine home visits, symptom management, and emotional support.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Coordinate patient admissions to in-patient settings, as needed, to ensure seamless continuity of care.
- Order and manage durable medical equipment, as required, to support patient comfort and well-being.
- Facilitate communication between patients, families, and healthcare providers to ensure informed decision-making.
- Document patient care and progress in accordance with agency policies and regulatory requirements.
- Graduation from an approved School of Nursing.
- Current LPN licensure in the State of Ohio and eligibility to obtain licensure in the State of Indiana, or current LPN licensure in the State of Indiana and eligibility to obtain licensure in the State of Ohio.
- Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing.
- Hospice experience preferred.
