Position Overview

The Senior Care Management and Wellness Coach will play a vital role within our Operations team, delivering exceptional service and support to our policyholders. This position is integral to a pre-claim long-term care insurance initiative aimed at promoting healthier living and providing education to caregivers and policyholders alike.

Key Responsibilities

  • Conduct ongoing assessments with policyholders and their families to establish goals and action plans following an initial comprehensive evaluation.
  • Research and identify local resources to assist policyholders in achieving their objectives.
  • Provide guidance on the use of daily living aids and facilitate next steps in the program as needed.
  • Utilize strong communication skills to build rapport with policyholders and their families, ensuring a supportive and empathetic interaction.
  • Analyze assessment results to recommend appropriate interventions and solutions tailored to individual needs.
  • Assist policyholders in overcoming common barriers to aging in place, adjusting goals as necessary to maintain motivation and engagement.
  • Deliver educational content on medical conditions, community resources, and caregiving strategies in an accessible manner.
  • Work independently while collaborating with an interdisciplinary team, managing multiple responsibilities effectively.

Qualifications

  • A genuine passion for working with older adults and their families.
  • A minimum of two years of supervised experience in care management.
  • Five to seven years of professional experience in relevant fields such as home care, hospice, or community services.
  • A degree in a related field or equivalent experience.
  • Strong knowledge of care management practices and community resources.
  • Excellent communication and customer service skills.
  • Ability to meet productivity standards while maintaining high-quality service.

Preferred Qualifications

  • Advanced degree or certification in gerontology or a related field.
  • Experience in patient advocacy and wellness coaching.
  • Familiarity with CRM systems and telephone case management.
