VITAS Healthcare is seeking a skilled Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. As an LPN with VITAS, you will play a vital role in providing high-quality, patient-centered care to individuals with terminal illnesses.
Key Responsibilities- Administer nursing care according to established plans of care, in accordance with state Nurse Practice Acts.
- Evaluate patient/family needs and assist in the preparation and execution of interdisciplinary plans of care.
- Perform selected acts in the care of terminally ill patients under the supervision of a registered nurse.
- Monitor patient reactions and report significant findings or changes in condition to registered nurses, team managers, or physicians.
- Document reports in clinical notes and maintain accurate documentation.
- Establish therapeutic relationships with patients/families and invite their participation in the plan of care.
- Initiate and perform nursing protocols based on patient needs, including positioning, mobilizing, bowel and bladder elimination, skin and wound care, personal hygiene, and communication.
- Participate in patient/family teaching during daily care, focusing on understanding the illness and its treatment, maintaining activities of daily living, self-care, and self-medication.
- Plan and carry out programs of professional growth, including participating in in-service programs and community educational offerings.
- Cooperate with education and research activities as assigned.
- Graduate of an approved school of Practical/Vocational nursing.
- Currently licensed to practice nursing in the state where the VITAS program is located.
- At least one year of satisfactory nursing experience, with at least one year of LPN/LVN experience in a hospital on an adult medical surgical unit.
- Equivalent experience may be considered.

While performing the duties of this position, the employee is regularly required to:
- Assist in lifting and transferring patients, as required.

- Ability to adjust focus.
The employee will be required to sit and stoop, kneel, and crouch.
