About the Role

Covenant Health is seeking a highly skilled Registered Nurse Care Manager I to join our team. As a key member of our healthcare team, you will be responsible for coordinating patient care, promoting quality improvement, and serving as a clinical resource and consultant to the healthcare team.

Key Responsibilities
  • Develop and implement comprehensive care plans for patients, taking into account their unique needs and circumstances.
  • Collaborate with healthcare team members to ensure seamless care coordination and communication.
  • Identify and mitigate variances in patient care processes, ensuring timely and effective interventions.
  • Participate in quality improvement initiatives, analyzing data and implementing evidence-based practices to improve patient outcomes.
  • Provide education and support to patients and families, empowering them to take an active role in their care.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices, applying this knowledge to improve patient care and outcomes.
Requirements
  • Current Tennessee RN license.
  • Three years of experience as a Registered Nurse, with a minimum of two years in areas of assigned responsibility.
  • Five years of RN experience is preferred.
