Key Responsibilities:

  • Provide physical therapy services to patients in their homes, focusing on rehabilitation and recovery.
  • Develop and implement individualized treatment plans based on patient assessments.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Maintain accurate documentation of patient progress and treatment outcomes.

Position Requirements:

  • A degree in physical therapy from an accredited institution.
  • Current state licensure to practice as a physical therapist.
  • A minimum of two years of relevant experience, preferably in community or home health settings.
  • Strong verbal and written communication skills, along with excellent organizational abilities.
  • Current CPR certification.
  • Valid driver's license and access to a reliable, insured vehicle.
  • At least one year of experience in home health or a similar environment.
