Position Overview: As an Advanced Care Provider (Physician Assistant/Nurse Practitioner) specializing in Aftercare, you will serve as a dedicated clinician, providing essential support to members during medical emergencies.
Work Schedule: This is a Per Diem role with hours from 8 AM to 8 PM, offering a hybrid work opportunity.
Key Responsibilities:
- Conduct follow-up assessments with members/patients after their visits, whether in-person, at home, or through telemedicine, to evaluate their health status.
- Perform comprehensive medical evaluations, including symptom assessment, medication prescriptions, and ordering necessary laboratory tests and imaging.
- Develop differential diagnoses and treatment plans tailored to urgent and emergent care needs outside of traditional medical clinic environments.
- Document all clinical evaluations and treatment strategies in the electronic health record system.
- Facilitate video telemedicine consultations when appropriate.
- Assist patients with hospital admissions and transfers as required.
- Provide compassionate care, addressing patient inquiries with patience and professionalism, utilizing empathetic communication.
- Collaborate with the patient's primary care physician and specialists to discuss assessments and coordinate follow-up care.
- Engage with our medical partner physicians and patients' primary care providers to ensure seamless communication.
- Work closely with the Medical Director and clinical team to uphold policies and deliver high-quality care in an innovative environment.
- Perform additional duties as assigned.
Qualifications:
We value diverse backgrounds and experiences, but ideally, candidates will possess:
- A Bachelor's Degree is required.
- Graduation from an accredited Physician Assistant or Nurse Practitioner program is essential.
- A valid state license to practice as a Physician Assistant or Nurse Practitioner.
- 2-3 years of relevant experience in emergency departments or urgent care settings.
- A DEA license is mandatory.
- Proven track record of delivering excellent clinical care and ensuring patient satisfaction in emergency and urgent care environments.
