About the JobThis is a unique opportunity to work as a Registered Nurse Case Manager at Del Sol Medical Center. As a member of our team, you will be responsible for coordinating patient care and promoting quality outcomes.
You will provide program orientation to patients, families, and caregivers, educating them on their care plan and available resources. Additionally, you will act as a coordinator of patient education, facilitating family conferences and interactions to ensure patient involvement in care planning.
The successful candidate will have a strong nursing background, with experience in case management or clinical nursing. They will possess excellent communication and analytical skills, with the ability to establish effective working relationships with healthcare professionals and patients alike.
In this role, you will work collaboratively with the multidisciplinary team to develop and implement patient care plans that meet individual needs and preferences. You will also be responsible for documenting patient progress, communicating effectively with healthcare providers, and identifying opportunities for process improvement.
