Responsibilities:
Assumes responsibility for delivering high-quality medication disease management services for assigned patient care areas. Demonstrates expertise in medication and disease management, as well as a thorough understanding of relevant treatment guidelines and medications for assigned clinical areas. Gathers necessary clinical data to inform medication therapy decisions and disease management. Assesses patient medication regimens for appropriateness, effectiveness, tolerability, safety, and potential adverse events. Makes clinically appropriate changes to patient regimens to maximize effectiveness and minimize complications. Provides educational materials and information to patients to enhance their understanding and active participation in care. Maintains accurate documentation of encounters in electronic health records and communicates relevant information to referring physicians and next providers of care.
Requirements:
Bachelor's Degree in Pharmacy (Required)
Minimum 2 years of related work experience (Required)
Licensed Pharmacist (Pennsylvania) - State of Pennsylvania
Estimated salary: $94,000 - $120,000 per year
