Key Responsibilities:
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Collaborate with healthcare providers, patients, and families to ensure comprehensive care and optimal outcomes.
- Conduct thorough assessments and identify opportunities for care improvement.
- Make informed decisions regarding patient placement and discharge planning.
- Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of care plans and goals.
- Stay up-to-date with industry developments and best practices in care management.
Requirements:
- Current RN licensure and 3 years of experience in Medical Surgical nursing.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
Preferred Qualifications:
- Bachelor of Science Degree in Nursing.
- Current de-escalation training or completion within 90 days of employment with annual recertification.
