Responsibilities:- Assist in planning, organizing, directing, and coordinating healthcare and professional services of the Licensed Home Care Service Agency.
- Oversee the opening of cases and ensure timely and efficient service delivery.
- Supervise and mentor aides to ensure they meet the highest standards of care.
- Develop and implement individualized care plans that meet the unique needs of each client.
- Collaborate with healthcare professionals to ensure seamless care transitions.
- Conduct regular assessments and evaluations to ensure client safety and well-being.
- Bachelor's degree in Nursing (preferred)
- Current NYS licensure as a Registered Professional Nurse
- Two years of experience as a supervising community health nurse or two years of home care nursing experience plus a master's degree in nursing, public health, business administration, or another health-related field
- Excellent communication and interpersonal skills
- Ability to read, analyze, and interpret documents, as well as speak effectively before groups
