Health Information Management Specialist

Job Summary
The Clinical Documentation Coordinator at Glens Falls Hospital is responsible for supporting the health information management processes within our healthcare system. This entry-level position focuses on the retrieval, preparation, and indexing of clinical documentation for our inpatient and outpatient facilities. The successful candidate will ensure compliance with legal, regulatory, and organizational standards while maintaining the integrity and confidentiality of patient information.
Key Responsibilities
* Prep, scan, and index incoming clinical documentation from internal providers and external referrers in accordance with departmental policies and procedures.
* Meet productivity and quality benchmarks and scorecard expectations set by HIM Operations Manager.
* Retrieve and sort through clinical documentation from departments when directed by HIM leadership team.
* Identify contents of incoming paper and electronic clinical documentation to appropriately route documents to physician, nurse, referral team, or other department in accordance with department policies and procedures.
* Collaborate with team to enhance data accuracy and integrity of medical record.
* Prepare clinical documentation to be scanned by reconciling encounters, orders, referrals, etc. Follow up on incomplete records per departmental reconciliation guidelines.
* Assist in the collection and reporting of quality metrics related to PCMH and CMS quality measures by adhering to departmental policies that target appropriate mapping of the medical record (patient, encounter, or order level).
* Perform other tasks and projects assigned by HIM Leadership Team.
* Perform thorough review of all incoming documents via mail, fax, email, etc. and redirect to appropriate departments accordingly.
* Notify HIM leadership of any workflow or build issues to mitigate potential problems promptly.
* Identify documents that constitute the legal content of the patient's health record, in both electronic and paper formats, to ensure integrity of record.
* Assign appropriate document types and supporting identifiers such as date of service.
* Properly complete destruction of records once prep, scan, index, and QA processing has been fulfilled. Understand departmental record retention schedule and assist with tasks related to storing, retention, and purging patient records.
Requirements
* High school diploma or equivalent required. Associates degree in HIM or related field preferred.
* 1 to 2 years of experience in Health Information Management (HIM) or related work environment required. Will consider Associates degree in HIM with a minimum of 80 hours of HIM practical experience in a hospital setting in lieu of work experience.
* Familiarity with EHR systems, ideally EPIC and OnBase.
* Basic understanding of PCMH and CMS quality measures.
* Strong attention to detail and organizational skills.
* Excellent communication and teamwork abilities.

Salary Range
The expected base rate for this position is $17.29 to $29.16 per hour. Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.
