Job Title: Nurse, LPN

Job Summary:

We are seeking a skilled and compassionate Nurse, LPN to join our team at Rappahannock Area Community Service Board. As a Nurse, LPN, you will provide high-quality nursing care and support to individuals with intellectual and developmental disabilities in a group home setting.

Responsibilities:

  • Provide general nursing care, including medication management and administration.
  • Assist with scheduling and support psychiatric services.
  • Conduct routine examinations, such as measuring vital signs and blood glucose levels.
  • Dispense medications and maintain accurate records.
  • Record data, prepare case records, and maintain detailed activity reports.
  • Provide counseling and educational information on medication use, diet, and health maintenance.
  • Prepare medical correspondence and reports.
  • Maintain first aid supplies and respond to medical emergencies.
  • Order, inventory, and secure pharmaceuticals.
  • Attend team meetings, staff meetings, conferences, and in-service training.
  • Provide resident and family medication management and behavioral support training.
  • Observe, document, and report reactions to medication/treatments and significant behavioral incidents.
  • Assist with admission process and development of treatment plans.
  • Monitor drug count and maintain records.
  • Assist with personal hygiene and grooming.
  • Monitor, instruct, and assist residents with daily living skills, mobility, communication, and community interactions.
  • Record resident progress and communicate behavior to relieving and management staff.
  • Report resident behavior to other service providers, family members, legal guardians, and authorized parties.
  • Facilitate networking and support for residents engaged in volunteer, social, and leisure activities.
  • Provide overnight support and supervision to residents and monitor activities.
  • Participate in housekeeping and meal preparation activities.
  • Assist in behavioral interventions.
  • Transport or escort individuals to activities and to and from the program.

Requirements:

We are seeking an individual with an Associate's Degree or completion of a technical training program in Nursing and some professional experience in nursing and/or behavioral health serving the intellectually-disabled population. The ideal candidate will possess a State Certified Licensed Practical Nurse license, a valid Virginia Driver's License, and current First Aid and CPR certification.

Working Conditions:

This position requires the ability to work calmly in stressful situations, establish and maintain effective working relationships with agency clinicians and staff, and operate a vehicle. The ideal candidate will also possess the ability to observe, assess, and record individual behaviors and functioning.
