Job Summary:

We are seeking a skilled Clinical Screener to join our team at Marquis Health Consulting Services. As a Clinical Screener, you will be responsible for assessing and evaluating referrals for clinical appropriateness for admission to our network of skilled nursing and rehabilitation centers.

Key Responsibilities:

• Assess and evaluate referrals for clinical appropriateness for admission to our network of skilled nursing and rehabilitation centers.
• Communicate directly with admissions/intake and clinical and administrative leadership at centers to support the admission process.
• Complete a clinical screen, identify applicable equipment needs, and review medications to ensure sound fiscal decision-making around acceptance.
• Expedite response to referral sources and optimize care transitions.

Requirements:

• LPN/RN licensure
• Ideal but not required experience within a skilled nursing center and working knowledge of capabilities
• Demonstrated technical skill and ability to navigate and communicate within referral platforms and SNF-based Electronic Medical Records
