Job Overview:
Rutgers, The State University of New Jersey, seeks a dedicated Health Care Case Manager for the Child Health Program within the School of Nursing. This role is critical in supporting the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care needs of children in out-of-home placement.

About the Role:
This position involves assessing children's health care status and needs, collaborating with medical professionals, caregivers, and other community service providers to develop comprehensive care plans. The ideal candidate will have strong communication skills, attention to detail, and experience working with vulnerable populations.

Responsibilities:
* Assist children in enrolling in managed care entities and identifying medical homes (primary care providers)
* Assess children's health care status and needs through reviewing medical records and interviewing past providers and caregivers
* Develop and implement individualized health care plans in collaboration with medical teams and caregivers
* Ensure timely EPSDT visits and specialty appointments, reviewing reports, and forwarding necessary documents to medical homes

Requirements:
* Bachelor's degree in Nursing or a related field required;
* Minimum 1-2 years of pediatric nursing experience or equivalent required;
* Current licensure as a Registered Nurse in New Jersey and certification in American Heart Association Basic Life Support for Healthcare Providers required;
* Vehicle and valid New Jersey license for off-site visits;
