About the Role:

We are seeking a dedicated Healthcare Navigator to join our team at St. Catherine's Center for Children.

The successful candidate will work closely with children, families, and caregivers to provide comprehensive care planning, ongoing care coordination, and community linkage services.

Key Responsibilities:

  • Conduct comprehensive assessments of healthcare needs and social determinants of health using designated health assessment tools and the NYS CANS assessment.
  • Develop and implement individualized care plans in collaboration with interdisciplinary teams.
  • Provide education and support to families on health management and disease prevention.
  • Coordinate with healthcare providers, social support agencies, and community resources to ensure seamless care delivery.
  • Maintain accurate records and communicate effectively with stakeholders.

Requirements:

  • Bachelor's degree in psychology, sociology, counseling, or a related field.
  • Minimum 2 years of experience in human services, preferably in child welfare, mental health, substance abuse, or physical health.
  • Proficiency in Microsoft Office Suite and Google Business Workspace Suite.
  • Experience with AWARDS/NYS Connections is an asset.

What We Offer:

  • $68,000 - $80,000 per annum, depending on qualifications and experience.
