Job Summary

We are seeking a dedicated and compassionate Certified Home Health Aide (CHHA) to join our team in the Philadelphia, PA region. As a CHHA, you will be responsible for providing personalized care to a census of 5-6 patients, assisting with daily living activities, and reporting any changes in patient condition to the nurse.

Key Responsibilities
  • Provide or assist with ADL activities, including personal hygiene, grooming, and skin care.
  • Assist with toileting, positioning, and transfer activities, as well as frequent turning and positioning of bed-bound residents.
  • Assist with prescribed exercise and ROM activities taught by the Hospice Nurse.
  • Perform light housekeeping duties, including changing bed linen and patient laundry.
  • Feed and assist with meals.
  • Report any changes in patient condition to the Nurse.
Requirements
  • Valid PA CHHA certification.
  • Valid Driver's License and reliable mode of transportation.
  • Authorized to work in the United States.
  • Full vaccination against Covid-19.
