Job Overview

We are seeking a skilled Registered Nurse to join our Home Health team in Lake Zurich, Illinois.

About the Role:

  • This is a full-time position requiring 40 hours of work per week.
  • The successful candidate will provide home care services to patients and establish strong relationships with them and their caregivers.
  • This role involves rotating on an on-call schedule and working one summer and one winter holiday.

Key Responsibilities:

  • To provide high-level nursing care to patients in their homes, preventing rehospitalization and promoting independence.
  • To document patient information and connect with the care team through point-of-care documentation.
  • To make visits to private residences, which may not be accessible to people with disabilities.

Requirements:

  • A graduate from an accredited school of nursing.
  • One year of medical-surgical experience in an acute or subacute care facility.
  • A current Illinois RN license.
  • A current CPR certification.
  • A commitment to earning 10.0 continuing education credits annually.
  • An ability to lift up to 35 pounds without assistance.
