The salary range for this position is $120,000-$145,000 per year, depending on experience.

We are seeking a highly skilled Family Nurse Practitioner or Physician Assistant to join our team at the Southwest Center. This individual will be responsible for providing patient-centered healthcare to individuals and families in a diverse and inclusive environment.

The ideal candidate will have a strong understanding of health promotion and disease prevention, as well as experience working with patients from various backgrounds. They will also possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

About the Role

  • Provide high-quality medical care to patients, including preventative and primary care services.
  • Develop and implement individualized plans of care for patients, taking into account their unique needs and circumstances.
  • Collaborate with multidisciplinary teams to ensure comprehensive care for patients.
  • Maintain accurate and up-to-date records of patient interactions and progress.
  • Promote healthy behaviors and lifestyles among patients, focusing on prevention and wellness.

Requirements

  • CURRENT Arizona license from the State Board of Nursing for Advanced Nursing Practice, OR the Arizona Regulatory Board of Physician Assistants.
  • Ability to establish and maintain effective working relationships with patients, staff members, and agency leadership.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Excellent written and verbal communication skills, with the ability to articulate complex ideas and concepts.
  • Proficiency in Microsoft Office software applications and electronic medical records systems.
