Job Summary:
Peak Resources, a leading provider of nursing home, memory care, and assisted living services, is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As a valued member of our staff, you will play a critical role in delivering high-quality patient care and contributing to the overall well-being of our residents.
Key Responsibilities:
- Implement and maintain established health care plans for residents, working closely with Registered Nurses (RNs) and other healthcare professionals.
- Participate in ongoing data collection and contribute to the development of individualized care plans.
- Provide exceptional patient care, ensuring the physical, emotional, and social needs of residents are met.
- Collaborate with the interdisciplinary team to achieve optimal patient outcomes.
Requirements:
- Current, unencumbered LPN license in the state of practice.
- Current BLS CPR certification, including in-person skills assessment.
- Excellent interpersonal and communication skills, with the ability to work effectively with residents, families, and healthcare professionals.
- Proficient computer skills and ability to navigate electronic medical records.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
Preferred Qualifications:
- One to two years of experience in a hospital or nursing facility setting.
