As a valued member of our staff, you will play a critical role in delivering high-quality patient care and contributing to the overall well-being of our residents.

Key Responsibilities:

  • Implement and maintain established health care plans for residents, working closely with Registered Nurses (RNs) and other healthcare professionals.
  • Participate in ongoing data collection and contribute to the development of individualized care plans.
  • Provide exceptional patient care, ensuring the physical, emotional, and social needs of residents are met.
  • Collaborate with the interdisciplinary team to achieve optimal patient outcomes.

Requirements:

  • Current, unencumbered LPN license in the state of practice.
  • Current BLS CPR certification, including in-person skills assessment.
  • Excellent interpersonal and communication skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Proficient computer skills and ability to navigate electronic medical records.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.

Preferred Qualifications:

  • One to two years of experience in a hospital or nursing facility setting.
