Job Overview

As a Primary Care Nurse Practitioner with National Recruiters, you will play a vital role in conducting comprehensive health risk assessments for Medicare Advantage members in their homes.

This opportunity focuses on evaluating patients' physical, mental, and social needs without the responsibility for ongoing care.

Responsibilities:
  • You will conduct in-home health risk assessments on Medicare Advantage members in Stafford Springs and throughout northeastern Connecticut.
  • Identify and document all current and chronic health conditions, assessing physical, mental, and social needs.
  • Provide patient education and counseling on identified conditions and screening procedures.
  • Record assessment findings using a provided iPad - no prescriptions, follow-ups, or on-call duties required.
Requirements:
  • An active and unrestricted Nurse Practitioner license is required.
  • A minimum of 3 years of patient care experience (preferably in primary care, adult or geriatric settings and with EMR proficiency) is necessary.
  • A minimum commitment of 18 hours per week, including one full 8-hour day (Sunday-Saturday), is expected.
  • Ability to work independently and autonomously is essential.
  • Strong empathy, compassion, and interpersonal skills are required.
  • Excellent verbal and written communication abilities are necessary.
Compensation and Benefits:
  • The compensation for this position is competitive and based on per-patient assessment.
  • Mileage reimbursement at $0.67 per mile after the first 20 miles is available.
  • Paid training and compensation for patient no-shows is offered.
  • No responsibility for ongoing patient care after the initial visit is required.
  • No direct communication with the patient's PCP, all notes are submitted electronically.
  • A collaborating physician is assigned, and malpractice insurance is fully covered.
  • The estimated annual salary for this position is $60,000-$80,000 based on the location and requirements.
