About the Position
This full-time position offers a unique opportunity to join a dynamic team of healthcare professionals committed to delivering exceptional patient care. As a Speech Language Pathologist with CenterWell Home Health, you will have the chance to work closely with patients, family members, and healthcare providers to assess communication needs and develop effective treatment strategies. Your expertise in speech and language therapy will enable patients to prevent, identify, evaluate, and minimize the effects of communication disorders and conditions.

Main Duties
• Conduct comprehensive evaluations of patients' communication abilities, identifying strengths and areas for improvement
• Develop and implement individualized treatment plans to address patient goals and objectives
• Deliver high-quality speech and language therapy services to patients in their homes, focusing on prevention, identification, evaluation, and minimization of communication disorders
• Maintain accurate and detailed records of patient progress and treatment plans
• Collaborate with interdisciplinary care teams to ensure comprehensive patient care and optimize outcomes

Qualifications
To be considered for this position, you must meet the following requirements:
• Certification in Speech Language Pathology (CCC-SLP) granted by ASHA
• Current and unrestricted license to practice in the state where services will be provided
• At least six months of experience as a speech therapist/speech language pathologist
• Home Health experience is highly desirable
• Excellent communication and organizational skills, with ability to work independently and as part of a team
