Position Overview:

As a Care Coordination Specialist, your primary responsibility will be to conduct assessments, develop plans, implement strategies, and oversee all care coordination activities. Your objective is to evaluate and meet the healthcare needs of members, ensuring their overall health through a holistic care management approach.

Key Qualifications:

  • Education: Bachelor's degree in Social Work or a related discipline (Nursing degrees are not applicable).
  • Experience: Proven experience in care coordination; experience in long-term care is advantageous.
  • Skills: Proficient in Microsoft Office Suite, particularly Excel. Strong communication skills (both verbal and written), with the ability to manage multiple tasks and thrive in a dynamic environment.
  • Preferred: Bilingual proficiency in Spanish and English.

Additional Information:

  • Training will be conducted remotely using virtual platforms.
  • Candidates will be required to travel approximately 75% of the time within the region, visiting members in various settings.
  • This position is primarily remote with potential for future onsite responsibilities.
