About the Role

We are seeking a highly skilled and compassionate Licensed Practical Nurse to join our team at All Ways Caring HomeCare. As a key member of our home health care team, you will provide direct and indirect care to clients in their home environment, promoting their independence and well-being.

Key Responsibilities
  • Provide skilled nursing care and treatment to clients, following physicians' orders and plans of care
  • Accurately document nursing actions, individual health status, and communicate with family and physicians
  • Document progress and outcomes for established goals, and inform physicians, case managers, and families of changes in clients' medical conditions and needs
  • Administer medications and manage medication regimens as needed
  • Provide health teaching and assist clients in learning self-care techniques and safety measures
  • Conduct nursing assessment visits as needed
  • Report incidents and follow policies and procedures according to operational, state, and regulatory standards
Requirements
  • Degree in Nursing from an accredited school
  • Experience in administering medications
  • One or more years of nursing experience preferred
  • Home health care experience preferred
  • Current and unrestricted license in the state of operation
  • CPR professional certification
  • Basic clinical skills
  • Professional appearance and demeanor
  • General knowledge of computers and ability to learn industry-specific software and documentation applications
  • Excellent oral, written, and interpersonal communication skills
  • Excellent organizational and time management skills
