About the Role

We are seeking a skilled Occupational Therapist to join our team at AT Home Care & Hospice. As a Home Health Occupational Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Evaluate patient's functional status and home environment to identify areas for improvement
  • Develop and implement personalized treatment plans to enhance patient function and independence
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination
  • Supervise Occupational Therapy Assistants and participate in discharge planning
  • Ensure compliance with state and federal regulations and guidelines
Requirements
  • Graduate of an occupational therapist education program accredited by ACOTE with current licensure to practice in state of operation
  • 2+ years occupational therapy experience, with home health experience preferred
  • Commitment to clinical and documentation excellence
