Responsibilities:
  • Conduct home visits to assess and treat patients with physical disabilities or impairments.
  • Develop and implement individualized care plans to improve patients' mobility, strength, and balance.
  • Collaborate with nursing staff to ensure comprehensive care and participate in multidisciplinary care planning.
  • Educate patients and their families on rehabilitative care and activities to promote independent living.
  • Accurately document patient care and services provided.
Qualifications:
  • Current license as a Physical Therapist in Massachusetts.
