Work Schedule:
Monday to Friday, 8-hour shifts, with rotating weekends.
Key Responsibilities:
- Accurately document evaluation results, ensuring all essential elements are included as per departmental policy.
- Complete daily progress notes that reflect patient responses to treatment, actual performance, and assessments relevant to the treatment plan, including discharge needs and summaries when necessary.
- Prepare discharge notes summarizing the treatment course, progress towards goals, discharge requirements, follow-up recommendations, and patient/family education.
Ensure documentation is clear, concise, and legible within established timeframes to facilitate continuity of care and support reimbursement processes.- Provide evidence of patient and family/caregiver education.
- Proactively communicate with families, physicians, and other healthcare team members to anticipate and address the special needs of patients and families in a timely manner, thereby enhancing treatment plans.
- Offer educational materials and methods tailored to the patient's understanding and motivation, involving families/caregivers in the education and training process.
Qualifications:
- New graduate physical therapists are encouraged to apply. One year of experience in acute care is preferred.
Educational Background:
- Bachelor of Science in Physical Therapy; Master's Degree is preferred.
Certifications:
- BLS certification through the American Heart Association is required.
Licensure:
- A valid Virginia License is required upon commencement of employment.
