About the Job

We are looking for an experienced RN with excellent clinical skills and a passion for delivering exceptional patient outcomes. As a Case Management RN Specialist, you will work closely with our multidisciplinary team to assess patient needs, develop individualized care plans, and coordinate services to ensure seamless transitions between care settings.

Key Responsibilities
  • Assess Patient Needs: Conduct comprehensive assessments of patients' physical, emotional, and social needs to identify opportunities for improved care coordination and service delivery.
  • Develop Care Plans: Collaborate with patients, families, and healthcare providers to develop individualized care plans that address patients' unique needs and goals.
  • Coordinate Services: Arrange for necessary services, including medical consultations, diagnostic testing, and referrals to specialists or community resources, to ensure timely and effective care delivery.
  • Communicate Effectively: Develop and maintain strong relationships with patients, families, and healthcare providers through clear, concise communication and empathetic listening skills.
Requirements
  • Graduate of an accredited nursing program with a current RN license in at least one state or territory within the US.
  • Minimum 2 years of experience in case management, critical care, or other relevant specialties.
  • Strong clinical knowledge and expertise in assessing patient needs, developing care plans, and coordinating services.
  • Excellent communication, interpersonal, and problem-solving skills.
