As a key member of our healthcare team, you will play a vital role in coordinating psychosocial assessments and discharge planning, ensuring the best possible outcomes for our patients.
Key Responsibilities:
- Coordinate psychosocial assessments and discharge planning for patients receiving inpatient psychiatric services.
- Act as a resource to patients, utilizing knowledge of community and healthcare resources to address identified needs.
- Facilitate quality resource utilization, including length of stay and level of care.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Provide leadership and psychosocial support to patients and their families.
Requirements:
- Master's degree in social work, counseling, or a related field.
- Previous social work experience, preferably in a healthcare setting.
- Maryland Social Work licensure (LMSW or LCSW-C) or equivalent.
