About the Role

We are seeking a skilled Nurse Practitioner to join our HouseCalls team in the West Region. As a key member of our team, you will conduct in-home assessments, perform evidence-based practice screenings, and communicate findings to inform member's PCP of potential gaps in care.

Responsibilities
  • Conduct comprehensive in-home assessments, including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and physical exam
  • Perform point of care testing and identify diagnoses to inform care management and active medical management
  • Communicate findings to inform member's PCP of potential gaps in care
  • Educate members on disease process, medication, and compliance
  • Identify urgent and emergent situations and intervene appropriately
Requirements
  • Active NP license or ability to obtain by start date
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and various dwelling conditions
  • Access to reliable transportation to travel up to 85% within licensed states
Preferred Qualifications
  • 1+ years of clinical experience in their highest level of education, clinical setting
  • Experience in Gerontology, Cardiology, Internal medicine, or Endocrinology
  • Home Health care or home visit experience
