Key Responsibilities

  • Provide emotional support and guidance to patients and their families, addressing issues such as parenting, substance abuse, stress management, and mental health concerns.
  • Collaborate with healthcare teams to develop and implement care plans that meet the unique needs of each patient.
  • Assess patients' mental health and emotional well-being, identifying areas where support is needed and developing strategies to address these concerns.
  • Develop and maintain relationships with community resources and services, ensuring patients have access to the support they need.
  • Document patient interactions and progress, maintaining accurate and up-to-date records.

Requirements

  • Master's Degree in Social Work
  • No prior experience required, but recent work experience in healthcare is preferred.
  • Strong communication and analytical skills, with the ability to work independently and as part of a team.
  • Ability to maintain organization and prioritize tasks in a fast-paced environment.
  • Eligibility to sit for and pass the test of certification as a Certified Case Manager (CCM) or Accredited Care Manager (ACM) is preferred.
