High Intensity Case Manager, RN

We are seeking a highly skilled and experienced Registered Nurse (RN) to fill the role of High Intensity Case Manager. As a key member of our team, you will be responsible for effectively managing a panel of high-risk members to ensure they receive the appropriate level of care.
Key Responsibilities- Conduct thorough assessments to identify individual needs and develop a specific care management plan to address objectives and goals.
- Collaborate with healthcare providers, community-based programs, and behavioral health providers to ensure seamless coordination of care and services.
- Monitor and evaluate the effectiveness of care management plans and modify as necessary to minimize unnecessary utilizations, admissions, and readmissions.
- Interface with Medical Director and attend Interdisciplinary Team (IDT) meetings as required.
- Conduct outbound calls to assigned high-risk case-managed members and provide in-person visits as needed to facilitate care.
- At least two (2) years' experience in the medical field, with one (1) year experience in a case management role.
- Knowledgeable in NCQA requirements and certified in case management (CCM) preferred.
- RN/LVN unrestricted active license required.
