Key Responsibilities:
• Provide 1:1 care for clients, following their individualized plans of care and promoting their dignity and independence.
• Monitor clients' vital signs and report any abnormal findings to the case manager and physician.
• Administer medications, feedings, oxygen, and other treatments as needed, and document the results of these interventions.
• Complete routines established by other care professionals, such as physical therapists, occupational therapists, and speech therapists.
• Ensure infection control policies are maintained at all times and report any possible communicable diseases or infections to the case manager and physician.
• Accurately document nursing actions and communicate with family members, case managers, and physicians about client progress and outcomes.
• Order supplies and medications as needed to ensure adequate stock levels.
• Monitor incidents involving clients and ensure incident reports are completed timely and accurately.
• Ensure all medical equipment is maintained and report any faulty equipment to the Durable Medical Equipment team for repair.
Requirements:
• Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
• Knowledge of individual service plans, including understanding of client rights and health needs.
• Proof of current licensure as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
• CPR certification.
• Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
• In-depth understanding of patient care policies and regulations.
• Familiarity with symptoms and signs of improvement for several medical conditions.
