As a skilled Care Coordinator, you will play a vital role in providing patient-centered care to individuals in the comfort of their own homes. Avera@Home Yankton, a leading healthcare provider, seeks a dedicated Registered Nurse (RN) to join our team as a Home Health Care Case Manager.
About the Role- You will be responsible for developing and implementing comprehensive care plans that cater to the unique needs of each patient, working closely with an interdisciplinary team to ensure seamless care coordination.
- Your expertise in nursing will enable you to assess patients' health status, identify areas for improvement, and implement evidence-based interventions to enhance their quality of life.
- In this dynamic role, you will have the opportunity to work with patients from diverse backgrounds, fostering strong relationships built on empathy, compassion, and trust.
- Conduct thorough assessments of patients' physical and emotional needs, identifying opportunities for growth and development.

- Bachelor's degree in nursing is preferred, but not mandatory.
- You must possess a valid driver's license and meet the requirements for multi-state licensure.
- A competitive hourly rate between $29.50 - $44.25, dependent on experience.
