Job Summary

We are seeking a Licensed Practical Nurse (LPN) to join our team as a Home Health Care Specialist. In this role, you will provide hands-on nursing care to clients in their homes, working closely with other healthcare professionals to ensure the best possible outcomes.

Key Responsibilities

  • Provide skilled nursing care to clients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of clients' needs and develop individualized care plans to meet those needs.
  • Work collaboratively with other healthcare professionals, including physicians, therapists, and social workers, to coordinate client care.
  • Communicate effectively with clients and their families to ensure they understand their care plans and any changes to those plans.
  • Maintain accurate and up-to-date records of client care, including progress notes and treatment plans.

Requirements

  • Currenth valid LPN license in the United States.
  • Graduation from an accredited LPN program.
