Job Title: Physical Therapist - Days

We are seeking a highly skilled and dedicated Physical Therapist to join our team at HexaQuEST Health, Inc. as a Physical Therapist - Days. As a Physical Therapist, you will play a vital role in the rehabilitation and recovery of our patients, providing expert care and guidance to help them achieve their full potential.

Job Summary:

The Physical Therapist - Days will be responsible for evaluating, planning, directing, and administering physical therapy modalities of treatment as prescribed by a licensed physician. You will work closely with our multidisciplinary team to provide comprehensive care to patients, including pediatric, adolescent, geriatric patients, and the general patient population.

Key Responsibilities:
  • Evaluate patients to determine their physical therapy needs and develop individualized treatment plans.
  • Administer physical therapy treatments and modalities, including exercises, stretches, and manual therapy techniques.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Conduct assessments and reassessments as per policy to ensure optimal patient outcomes.
  • Develop and implement teaching plans to educate patients and families on proper techniques and self-care strategies.
  • Maintain accurate and detailed records of patient progress and treatment plans.
  • Collaborate with other healthcare professionals to ensure comprehensive care and address any concerns or issues that may arise.
Requirements:
  • Current State Physical Therapy Licensure in Illinois.
  • Graduate of an approved school of Physical Therapy or accredited college or university with a certificate in Physical Therapy.
  • Experience in all aspects of physical therapy, including pediatric, adolescent, geriatric patients, and the general patient population.
  • Current BCLS (CPR) certification.
  • Strong written and verbal communication skills.
  • Basic computer knowledge and proficiency in electronic medical records.
