We are seeking an experienced Licensed Practical Nurse (LPN) to join our team at the Good Samaritan Society in Holstein, IA. As a LPN, you will play a vital role in providing high-quality care to our residents and contributing to their overall well-being.

Job Summary

The Licensed Practical Nurse provides professional nursing care for residents of all ages in long-term care, under the supervision of a registered nurse, advanced practice provider, or physician. You will be responsible for delivering comprehensive care that addresses the physical and psychosocial needs of our residents.

Your Key Responsibilities

  • Provide direct patient care, administering medications, treatments, and other interventions as ordered by physicians or advanced practice providers.
  • Collaborate with healthcare professionals to develop and implement individualized care plans that meet the unique needs of each resident.
  • Conduct thorough assessments of residents, identifying areas of strength and weakness, and developing strategies to address these concerns.
  • Maintain accurate and up-to-date records of resident interactions, including documentation of medication administration, behavior, activity, and dietary habits.
  • Communicate effectively with residents, family members, and healthcare professionals to ensure seamless care coordination and resolve any issues promptly.
  • Participate in interdisciplinary team meetings to discuss resident care plans, share best practices, and address challenges.

Requirements

  • A graduate of an accredited practical nursing program with a current unencumbered LPN license.
  • Possess strong critical thinking, problem-solving, and communication skills.
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities simultaneously.
  • Familiarity with electronic medical records systems and other healthcare technologies.
  • Strong teamwork and collaboration skills, with a focus on building positive relationships with colleagues and residents.
