  • Develop and implement individualized assessment-based treatment plans.
  • Consult with team managers, statutory and voluntary agencies, and patient relatives to ensure compliance with patient treatment plans.
  • Participate in comprehensive psychosocial assessments and interdisciplinary rounds.
  • Provide individual, group, family, and crisis counseling.
  • Act as a liaison with patients, families, community support workers, and facilities to facilitate discharge planning.
  • Provide education and resources for patients and family members.
  • Supervise and/or train lower-level social workers, students, or interns.
  • Process paperwork, telephone calls, and complete clinical documentation.
  • Participate in continuing professional education, departmental meetings, and teaching.
