At TriStar Southern Hills Medical Center, we are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Inpatient Rehabilitation team. As an LPN, you will play a vital role in providing direct patient care under the supervision of a Registered Nurse and collaborating with the healthcare team to assess, plan, implement, and evaluate the plan of care.

Key Responsibilities:

  • Monitor, record, and report patient symptoms, reactions, and progress to the healthcare team.
  • Create and maintain accurate, detailed reports and records.
  • Collaborate with the healthcare team to assess, plan, implement, and evaluate the plan of care.
  • Perform nursing tasks as appropriate and safe under indirect supervision of the RN.
  • Reinforce individualized education for the patient, family, and/or caregiver throughout the continuum of care as directed by the RN.

Requirements:

  • Graduate of an accredited School of Practical Nursing.
  • Current licensure by the Tennessee Board of Nursing.
  • Current BLS Certification.
