Job Summary:

The Case Manager will work with adults with disabilities, older persons, and family caregivers to assess their needs and provide support to maintain their independence in the community.

Key Responsibilities:

  • Assess client needs and develop a service plan to maintain independence
  • Conduct electronic versions of CARE comprehensive functional assessments and reassessments
  • Develop and implement a plan of care listing services recommended and describing the division of tasks between the Case Manager, support network, and the client
  • Determine whether resources are appropriately meeting the needs of the client and institute case coordination with community contacts and resources
  • Authorize services available in the client's Care Plan and authorize payment for services utilizing the ProviderOne payment system
  • Access and utilize various DSHS data systems, including the Automated Client Eligibility System (ACES) and the ProviderOne payment system
  • Provide termination planning when the client's informal support systems are operating adequately to meet client need or the client no longer meets case management target population criteria
  • Attend Case Management staff meetings, case reviews, and inter-agency staffing

Requirements:

  • Bachelor's degree in Social Work, Social Science, Counseling, or related field
  • Knowledge of the principles and practice of social work, the aging process, and Washington State Aging Network service delivery system
  • Ability to understand and apply federal, state, and tribal policies & procedures and regulations applicable to confidentiality policies
  • Proficient with Microsoft Office Suite and ability to utilize personal and laptop computers, telephone, copy and fax machines, typewriter, and adding machine
  • Bilingual English/Spanish preferred
