- Collaborate with the treatment team to develop and implement individualized care plans
- Communicate effectively with staff and consumers to ensure seamless care coordination
- Monitor and report medication variances, ensuring timely follow-up and resolution
- Participate in in-service training sessions to enhance knowledge and skills
- Perform other duties as assigned by management
- Current LPN licensure in the state of Pennsylvania
- Ability to work collaboratively as part of a treatment team
- Excellent communication and interpersonal skills
- Strong organizational and time management skills
- Ability to maintain confidentiality and handle sensitive information
