TeamHealth is seeking a skilled Nurse Practitioner to join our post-acute care team in Moses Lake, Washington.

Job Summary:

We are looking for a highly motivated and experienced Nurse Practitioner to provide primary care services to patients in a post-acute setting. As a member of our team, you will have the opportunity to work autonomously, manage a patient panel, and collaborate with interdisciplinary teams to optimize patient outcomes.

Responsibilities:
  • Provide primary care services to patients in a post-acute setting
  • Manage chronic disease, diagnose illnesses, and implement plans of care
  • Order and interpret lab results, prescribe and monitor medications
  • Collaborate with interdisciplinary teams to optimize patient outcomes
  • Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
Requirements:
  • 1+ years of experience in primary care
  • Active and unrestricted Washington NP or PA license
  • Active and unrestricted DEA
