LPN - Home Health Specialist

About the Role
We are seeking a skilled Licensed Practical Nurse to join our Home Health team at Providence. As a LPN, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.
Key Responsibilities
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and care coordination.
Requirements
- Graduate of a school of nursing.
- Current Washington State LPN licensure.
- 1 year of experience in an acute care setting, with a focus on home health or gerontology.
- National Provider BLS - American Heart Association certification.
- National Provider Identifier (NPI) number.
Preferred Qualifications
- 1 year of experience in home health care.
