We are seeking a highly skilled Family Nurse Practitioner to join our team in metropolitan Phoenix. This is an excellent opportunity for a motivated and compassionate healthcare professional to make a meaningful difference in the lives of our patients.
Job Summary
The successful candidate will provide advanced nursing care across the continuum of health services to meet the specialized physiologic and psychological needs of patients with acute, critical, and/or complex chronic health conditions. The Nurse Practitioner will work collaboratively with our multidisciplinary team to deliver high-quality patient care and achieve exceptional outcomes.
Key Responsibilities
- Perform initial patient screenings, patient history, and physical examinations to determine care plans and treat health conditions.
- Update patient health records and ensure accuracy at each appointment to maintain continuity of care and facilitate collaboration with other healthcare professionals.
- Analyze test results and imaging reports to determine the need for follow-up appointments and further treatment options.
- Provide ongoing care information, counseling, and guidance to patients with critical conditions.
- Conduct chronic health assessments to determine health conditions and develop treatment plans, including ordering labs, imaging, referrals to specialty, and prescribing medication.
- Order pharmacologic and non-pharmacologic treatments for direct management of acute and chronic illness.
- Respond to onsite emergencies until EMS arrives.
- Use medical training and expertise to respond to changing patient physiologic status.
- Assess response to therapeutic interventions and recommend continued or changed treatment plans.
- Directly supervise nurses and other care team members.
- Acknowledge legal and ethical professional responsibilities, such as patient management, documentation, and advance directives.
Requirements
- Board Certified Family Nurse Practitioner required (AANP, AANC).
- Graduate of Certified Nurse Practitioner Program (Master's, post-masters or doctoral preparation).
- Active and unrestricted AZ license required (for certification and APN number).
- Prefer more than 3 years of experience working as an NP in primary care setting.
- Pharmacy and DEA prescription privileges required.
- BLS certification required, with ability to use AED, ACLS, and PALS preferred and required for Urgent Care.
- Excellent team building skills.
- Good computer skills using MS Office Products (Word/Excel) and Electronic Health Records systems.
Compensation and Benefits
The estimated salary for this position is between $110,000 and $130,000 per year, depending on experience and qualifications. We offer a comprehensive benefits package, including medical, dental, vision, and 401(k) plan.
