Hospice Nurse

Job Summary:

As a Hospice RN, you will be responsible for providing high-quality patient care and ensuring that patients and their families receive the best possible support. You will work closely with the interdisciplinary team to assess patient needs, develop care plans, and implement interventions to improve patient outcomes.

Responsibilities:

Conduct thorough assessments of patients and their families to identify their needs and develop individualized care plans
Collaborate with the interdisciplinary team to implement care plans and provide ongoing support to patients and their families
Provide education and support to patients and their families on hospice care and related topics
Maintain accurate and up-to-date patient records and charts

Requirements:

Licensed as a registered nurse in the state of practice
2+ years of hospice experience preferred
Knowledgeable of quality assessment and assurance procedures
Reliable transportation and ability to travel
Valid driver's license and proof of insurance required
