Job Description

As a Registered Nurse in Home Health, you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician. You will coordinate with other healthcare professionals to achieve and maintain continuity of patient care.

Responsibilities

  • Make initial and comprehensive nursing evaluation visits to determine primary focus of care and develop a plan of care within state-specific guidelines.
  • Perform patient comprehensive assessments at designated time points and develop an appropriate plan of care.
  • Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer source.

Required Skills and Qualifications

  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
