We are seeking a skilled Licensed Social Worker to join our team. This full-time role offers a salary range of $80,000 - $105,000 per year, depending on experience.

Job Overview

This position involves coordinating care for patients with varying levels of need, collaborating closely with an addiction medicine provider.

  • Facilitate communication and coordination between patients, healthcare providers, primary care physicians, and specialty mental health services to ensure effective patient care.
  • Maintain accurate records of patient interactions and care coordination in the EHR, ensuring multi-disciplinary teams stay informed.
  • Identify and connect patients with necessary community resources and treatment programs.

Clinical Responsibilities:

  • Provide evidence-based therapies, such as motivational interviewing, behavioral activation, and CBT, to patients in individual and group settings.
  • Develop and implement group programming for patients with co-occurring disorders.
  • Offer support and guidance to patients during critical periods, tracking progress and addressing questions.

Qualifications:

  • Master's degree in Social Work from an accredited institution.
  • At least 1 year of experience in treating individuals with addiction through individual and group therapy.
  • Proficiency in Motivational Interviewing, Behavioral Activation, Problem-Solving Treatment, and CBT.
  • Ability to work flexible hours, including evenings.
