The Registered Nurse (RN) will provide care and services to a group of patients, visiting them in their homes, skilled nursing facilities, or assisted living facilities. As a contributing member of the interdisciplinary team, the RN will provide expertise on psych-social assessments, interventions, and community resources.
Responsibilities- Provide care and services to patients in their homes, skilled nursing facilities, or assisted living facilities.
- Conduct psych-social assessments and develop interventions to meet patient needs.
- Collaborate with the interdisciplinary team to develop and implement patient care plans.
- Work closely with patients and their families to ensure their needs are met.
- Manage multiple priorities and tasks, including documentation, meetings, and communication with healthcare professionals.
- Current Registered Nurse (RN) Licensure: Hold a valid RN license in good standing in the state(s) of practice.
- Teamwork and Communication Skills: Work effectively within a team environment to collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Time Management and Organization: Manage multiple priorities and tasks, including documentation, meetings, and communication with healthcare professionals.
- Electronic Medical Records System: Ability to learn and utilize an electronic medical records system.
- Reliable Transportation: Possess a valid driver's license and reliable transportation to travel across the service area.
