Job Summary:We are looking for a compassionate and skilled Nurse Practitioner to provide primary care services to patients in SNFs. The ideal candidate will have a strong background in clinical care, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Responsibilities:- Provide high-quality, patient-centered care to residents in SNFs
- Rounding and managing the clinical care of the population at the assigned SNF
- Prescribing medications and treatments for residents residing at SNFs
- Providing timely documentation of all patient care/encounters
- Available for phone/secure messaging consultations with facility nursing staff
- Using Theoria's proprietary electronic health record (EHR) system, ChartEasyTM
- Current licensure as a Nurse Practitioner in the state of practice
- Experience working in SNFs or post-acute care settings
- Strong clinical skills and ability to work effectively in a fast-paced environment
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
